Freedom of Information logo

The Freedom of Information Act 2000 gives a general right of access to all types of recorded information held by public authorities.

Finding and requesting information

Heatherwood and Wexham Park Trust publishes much information about itself on this web site notably through its publication scheme which is made up of classes of information.
If you cannot find what you are looking for on the site, please request information. You can do this by writing in to the FOI Manager at Wexham Park Hospital by:

(Please note: If you require access to patient notes, please follow this link to download the application form
Access to Health Records)

Response

Once you have submitted your enquiry, you will receive a response within 20 working days. Please let us know if your request is urgent. If we are unable to provide the information you want we will explain the reasons to you.

Should you wish to find out about the progress of your request, please ring the FOI office on 01753 633364 or send an email to foi@hwph-tr.nhs.uk.

Charges

There are no charges for requests that involve less than £450 of staff time but we may request reimbursement for the cost of photocopying, printing and postage. We would warn you in advance if a charge were to be made. This will give you the chance to confirm your request or not to proceed if you should not want to pay the fee. As a rough guide to what your request for information might cost, charges for printing and photocopying are 10p per page.

Vexatious or Repeated Requests

The Trust is not obliged to comply with a request for information if the request is vexatious. Where the Trust has previously complied with a request for information which was made by any person, it is not obliged to comply with a subsequent identical or similar request from that person unless a reasonable interval has elapsed between the two requests. The Trust will log all requests for information for monitoring purposes and will be able to identify repeated or vexatious requests.

Appeals

Should you be unhappy with the way in which your request has been dealt with, the reasons given for being unable to supply information, or any other aspect of the way the Trust is responding to Freedom of Information, please contact the Information Compliance and FOI Manager on 01753 633364 or send an email to foi@hwph-tr.nhs.uk.
